A workshop focusing on answering questions about the Vermont Ethics Network’s Advance Directive Short Form. If the form is completed during the workshop, participants may upload the form to the Vermont Advance Directive Registry for review and acceptance. Facilitated by Wendy Scott, VNA & Hospice of the Southwest Region Volunteer

Gifford Medical Center – Advanced Care Planning Fair

Please join us for Gifford’s annual Advanced Care Planning Fair where you can complete or update an advance directive and have questions answered around advance directives and long-term care planning.

Gifford staff will be on hand to answer questions and assist with paperwork. A number of community partners and service providers will also be present for you to interact with.
