Obtaining Patient Directives

The Vermont Advance Directive Registry (VADR) provides 24/7 access to patient advance directives for health care facilities and providers. By becoming an authorized user of the VADR, your staff can search the VADR for patient advance directives and download a PDF copy for entry into the medical record. If you are not an authorized user, but need to obtain a patient directive, the VADR has a 24/7 hotline service supported by the US Advance Care Plan Registry.

Important Note: Advance directives do NOT need to be registered to be legally valid. Please check with your patient and their friends and family to make sure you have the most up to date advance directive available.

Become an Authorized User

Health care providers and facilities in Vermont should have access to the Vermont Advance Directive Registry to help locate patient documents in an emergency and to support patients and community members in creating their advance directives. Vermont hospitals are required to have access to the Vermont Advance Directive Registry. 

Funeral homes and crematories may also apply for access. Funeral and burial instructions may be given in advance directive documents. Just like health care professionals, funeral directors must also do their best to follow the instructions in an advance directive.

Authorized providers agree to:

  • Protect the issued identification number issued. Limit access to the identification number to employees with a need to access the registry.
  • Train employees on the proper use of the registry and the registrants’ documents, and the obligation to report any unauthorized access or misuse of information to the Vermont Department of Health.
  • Inform the registry as soon as possible, in writing, of any change or proposed change to the information contained in the original Provider Access Application form submitted to the registry.
