The Vermont Advance Directive Registry (VADR) provides 24/7 access to registered advance directives for health care facilities, providers, and funeral homes. By becoming an authorized user of the VADR, your staff can search the VADR for patient advance directives and download a PDF copy for entry into the medical record. If you are not an authorized user, but need to obtain a patient directive, the VADR has a 24/7 hotline service supported by the US Advance Care Plan Registry.

Important Note: Advance directives do NOT need to be registered to be legally valid. Please check with your patient and their friends and family to make sure you have the most up-to-date advance directive available.
Health care providers and facilities in Vermont should have access to the Vermont Advance Directive Registry to help locate patient documents when needed and to support patients and community members in creating their advance directives. Vermont hospitals are required to have access to the Vermont Advance Directive Registry.
Funeral homes and crematories may also apply for access. Funeral and burial instructions may be given in advance directive documents. Just like health care professionals, funeral directors must also do their best to follow the instructions in an advance directive.
