Resetting your Account Access

Many Vermonters registered their advance directives with the VADR before it was common to include email addresses in accounts or to have home computers. It’s possible that you have been registered with the VADR, but have never logged into your account online. If you need to log in to add your email address for annual reminders, or to check your account and advance directive, you may need to reset your account access in order to log in.

Option 1: Resetting your account access online

Visit the VADR Log-in Page and try your username and password. If you don’t have this information, you can request your username with the ‘Forgot Your Username?’ button and/or reset your password with the ‘Forgot Your Password?’ button.

If you need to reset your password, you can use your Registration ID number (found on your wallet ID card) OR your Username to get a password reset link by email. Username and Registration ID are case-sensitive.

If you need a reminder of your Username, you can request your username information via email as well. You will need your Registration ID and an email address to receive an email with your Username.

Option 2: Phone/email support for account reset

We can also reset your account access at your request by phone or email. Please include your full name and date of birth, or your registrant ID in your request.
