Central Vermont Medical Center – Advance Directive Event

  • When? Wednesday, April 16th from 7:00 a.m. to 5:00 p.m
  • Where? South Lobby Entrance, CVMC Medical Center (130 Fisher Rd, Berlin, VT 05602)
  • What? Day-long tabling event! Staff and volunteers will be available to distribute free advance directive forms, answer your advance directive questions, assist with signing and witnessing documents, and get your completed directive on file with CVMC and the Vermont Advance Directive Registry. Stop by any time between 7:00 a.m. and 5:00 p.m.
