When? Wednesday, April 16th from 7:00 a.m. to 5:00 p.m
Where? South Lobby Entrance, CVMC Medical Center (130 Fisher Rd, Berlin, VT 05602)
What? Day-long tabling event! Staff and volunteers will be available to distribute free advance directive forms, answer your advance directive questions, assist with signing and witnessing documents, and get your completed directive on file with CVMC and the Vermont Advance Directive Registry. Stop by any time between 7:00 a.m. and 5:00 p.m.

On National Healthcare Decisions Day, join us for a heartfelt community dialogue on the vital difference an Advance Directive can make, for you and for your loved ones. We’ll engage with a local panel and community peers to explore one essential question: what does Living Well actually mean to each of us when faced with a major health challenge. All are welcome!
