April 16th is National Health Care Decisions Day and April 14th  – 19th is Vermont Advance Directive Week. This April, we invite you to join people across the country in making your future health care preferences known! Learn more about advance care planning activities in your area.

Addison County

    Advance directive forms and materials will be available and staff will be present to answer questions.

Caledonia County

    Northeastern Vermont Regional Hospital – Main Lobby
    Advance directive forms and materials will be available and staff will be present to answer questions (this table will be staffed part-time).

Chittenden County
